What website do I go to so I can complete my online registration?

Some medical facilities have patients access their Simple Admit form by clicking on a patient registration link on the facility website. However, you can also go to www.simpleadmit.com and enter the customized password that you received from your surgery center. This password is not case sensitive and allows you to access your medical facility’s customized form.

What is my password?

Your medical facility will provide a unique password. This password is not case sensitive and allows you to access your medical facility’s customized form. If you’re unsure what your medical facility’s password is, or for any password related issues, please contact us. Please be sure to include the name of the medical facility where you’ll be receiving care, along with your full name and date of birth so we can provide you with the correct password.

My password isn't working.

If the password you are entering is not working, here are a couple things to check:

  1. Some passwords contain a zero (0) and/or the letter (O). Make sure you didn’t accidentally type the wrong character in the password.
  2. Passwords will never include a period or any punctuation. Make sure you’re not entering any punctuation marks in the password.

If it’s still not working, please contact us. Please be sure to include the name of the medical facility you’ll be treated at, along with your full name and date of birth so we can provide you with the correct password.

How do I get back into the Simple Admit form I started so I can complete it?

No problem! All the information you entered was saved up to the point of the last time you clicked on “Save and Continue” on the online registration. Every form in Simple Admit has a unique access token. To get back into your form, you’ll need to enter your access token. If you don’t have your access token, it can be emailed to you by either your medical facility or by one of Simple Admit’s Patient Support Specialists. Please be sure to include the name of the medical facility you’ll be treated at, along with your full name and date of birth, and we will be happy to email your access token to you.

Do I need to complete another form if I was a previous patient at the facility?

If you completed a Simple Admit form at the same facility you’ll now be visiting again, you can access your previously submitted health history form with your access token. If you no longer have your access token, it can be emailed to you by either the medical facility or by one of our Patient Support Specialists. Please contact us and be sure to include the name of the medical facility you’ll be treated at, along with your full name and date of birth, and we will be happy to email your access token to you.

Can I print my form or save it as a PDF?

Yes, simply log in using your access token. Before opening your form, you will see options for printing your form. When presented with print options, you can elect to “print” the file as a PDF or save it to share with your other medical providers.

Why can’t I move to the next page in my Simple Admit form?

Please make sure all questions marked with an asterisk (*) are answered, and make sure the date fields are in the correct format (MM/DD/YYYY). On the first page, make sure your height/weight responses do not have decimal points as they must be whole numbers.

What time is my appointment/arrival time?

How much will my insurance pay?

How do I reach a nurse?
